No, I think Steven choose name "Quartz" because he thought that his mom was Rose Quartz. But now, because he know the truth, he can call himself Steven Diamond Universe. And about the birth certificate, I don't think Steven has one bacause his mom was an alien and I think he's not registred as a citizen. This is how Pearl couldn't get her driving license because she's an alien and she's not a citizen. However, it's certain that the other characters I mentioned don't have acts to confirm their name. So, their name should be the most explicit and clear, based on their hybrid nature, as I said above.
